Job Description

• Establish clients’ goals by assessing vocational needs; developing, monitoring, and evaluating vocational plans and progress; facilitating interdisciplinary approaches; monitoring client performance.
• Monitors cases by verifying clients’ attendance; observing and evaluating vocational plans and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
• Maintains clients’ records by reviewing case notes; logging events and progress.
• Communicates clients’ progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to applicable parties.

Key Requirements, Skills, and Qualifications

• Flexibility to work from both our San Bernardino and Rancho Cucamonga offices.
• At least 21 years of age.
• Bi-lingual (English & Spanish)
• A minimum of two years experience working with persons with disabilities or a BS or BA in a Social Science.
• Professional knowledge of vocational rehabilitation services.
• Ability to communicate effectively between agencies and support systems regarding client progress, needs, and employment goals.
• Computer proficiency in a professional office environment.
• Have a valid California Driver's License and Auto Insurance

Education

High school or equivalent (Required)

Experience

• Case management: 1 year (Preferred)
• Working with individuals with disabilities: 2 years (Preferred)
